Difference between Chronic Microangiopathic and Acute Microangiopathic changes

When one is looking at an MRI, how does the radiologist or neurologist determine the difference between an accute microangiopahic change and a chronic microangiopathic change. Is it possible for an area where there is chronic microangiopathic changes to result in a stroke by a traumatic event like a motor vehicle accident?
