Dizzy, twitching and spasms

Can anyone tell me what is wrong with me?

I am a 44 year old male. 4 months ago I had my first bout of dizziness preceded by exhaustion and momentary flashes when I sensed a swirling sensation. Dizziness lasted 10 days + sore ears and mild loss of balance. This happened 5 times - each time for about 10 days followed by a few days of relative normality. After about 3 weeks the lymph nodes on the back of my neck became enlarged. they are still larger than usual.

Then I got stiff sore legs. Then my arm muscles started to get stiff. I got muscle twitching all over my body - especially in my calf muscles and especially at rest or when I'm in bed. Most this is little twitches within the muscles, often repetitively (fasciculations I think). When I rest the front of my thighs feel tense.

Again this has happened in repeated episodes - each lasting a few days followed by a few days relative normality. I dont get dizzy any more. Each episode starts with tiredness and then I get mild muscle spasms - arms, legs, face, painful uncoordinated muscles, and then the twitching.

Currently my legs are heavy and sore (espcially in the mornings). Arms and legs feel a little uncoordinated but no apparent loss of strength. My throat is slightly inflamed and I have problems swallowing very small pieces of food (like rice). My ears still get a bit sore - especially my right ear (which is the side on which the problems in my leg and arm are most severe).

I've blood tests and CAT scans of brain and ears - but nobody can determine what is wrong with me. A neurologist said I had a normal neurological examination. An ENT doctor said he didnt think it was anytihng to do with my ears.
