Dizzy and Shoulder/Back Pain

It's been about 5 years I've had pain on my shoulders and my back. Just to sit for about 1 min. without lean on something makes my back hurts. And I have dizziness which never go away.  I also belch often. Also i get fatigue faster than ever before. The dizzy feels worse while I'm looking up or down.
I've had MRI on my head & back 5 years ago, but result says nothing is wrong.I also had endoscopy and the result says I have GERD but not severe, been taking the meds but not doing better. The USG show no problem on my gallbladder,kidneys,and liver.
I also have some anxiety and been taking the meds but not feeling better . My age now is 22. Could somebody tell me what is the disease related to my condition?

You need to have a more current MRI also need to see an orthopedic doctor and a neurologist between the 2 they can help you get diagnosed.
