Do i need a surgery based on my MRI report?

I have been suffering low back pain for several years. Based on my MRI report, my doctor said I need a surgery, since Physical therapy or Chiropractor won't help. But i do want a second opinion before i go for a surgery. So could someone help me with it?   The following is from my MIR report:

MRI LUMBAR SPINE
HISTORY: 30 year old male, back pain X 5 years, after heavy lifting.

A non-contrast MRI scan of the lumbar spine was obtained. The L4-L5 level shows mild disc space narrowing, disc degeneration. In addition, there is a moderate extruded left central disc herniation with significant impingement of the left L5 nerve root in the lateral recess.

The L5-S1 level shows minimal disc space narrowing, and disc degeneration. A tiny left central disc herniation abutting the left L5 nerve root is seen. There may be minimal nerve root impingement.

The lumbar spine is otherwise, unremarkable.

IMPRESSION;

L4-L5 and L5-S1 minimal degenerative changes. The L4-L5 moderate extruded left central disc herniation with significant left L5 nerve root impingement. L5-S1 tiny left central disc herniation abutting the left S1 nerve root with possible minimal nerve root impingement.

Without the ability to examine and obtain a history, I can not tell you what the exact cause of the symptoms is. However I will try to provide you with some useful information.

Your MRI is stating that you have severe impingement of the left L5 nerve root, which likely will not resolve with physical therapy and chiropractic therapy.  I would suggest that you follow up with another Orthopedic surgeon or neurosurgeon, both that specialize in spinal surgery.
