you can ask if you can take some pain medication beforehand like motrin...i didn't and i was very sore with lots of muscle twitching afterwards-maybe for a day...the shocks they give you for the NVC can be kinda painful when they crank up the current...the only time i actually really hurt during the EMG is when the doctor stuck the needle in near my spine...ouch...you may not have these problems, but i did...
this is no where near as painful/awful as a tilt-table test i had done once.

I doubt that running for 3 miles will skew the result, but you should call your neuro's office and ask to be sure.
If I remember correctly, they only use one needle and use it repeatedly. The pain is very minimal, but there is one spot in the thigh above the knee that can hurt quite a bit.
The pain in part depends on the skill of the neurologist and in part your individual physiology. Like I said before, the pain is very minimal and very brief, it hurts much less than getting a shot, and much of the time you won't be able to feel anything at all, so no worries. The needle is very long but it is extremely thin. I think it is thiner than an acupuncture needle.
