: My cousin is expecting and has MS.  She has been diagnosed with Cavernous Humangeoma.  Doctors are uncertain of the affect that labor and delivery will have on her.  The question is which is safer vaginal delivery or cesarean section?  What are the risks of both?  She is due to deliver the week of May 12, 1997.  Thank you for your help.

Wendy, A very interesting question. I have reviewed the medical literature for the past 30 years and found only 2 artcles that in any way address this topic. Both state that there is an increased risk of cavernous angiomas growing and bleeding during pregnancy, but none address the risk of bleeding during labor and delivery specifically much less the relative risks of vaginal vs. C-section delivery. Thus, I cannot answer your question as no one has ever studied this. I would leave the means of delivery up to the obstetrician and not let the angioma influence things one way or the other, as the fact that not a single reported case in the whole medical literature exists dicumenting a hemorrhage during delivery that I could find on Medline, suggesting that the risk, while theoretically greater, is very small in absolute terms. Again, however, I would proceed with whatever option the obstetrician is most comfortable with.
