Fasciculations in relaxed muscle vs. tense muscle

I have read in other forums that "A simple clinical rule is that fasciculations in relaxed muscle are never indicative of a motor system disease unless there is an associated weakness, atrophy or reflex change."  

My question is multi-part:
A.  Is this statement true?
B.  If true, what is the rule if the fasciculations occur in both relaxed AND tense muscles?

Hello.

Ther "rule" that you have mentioned here is true as far as theory is concerned.
There are yet cases where it the rule fails.

There are exceptions, always.

Fasciculations alone should not be the basis for any diagnosis.
