Iliolumbar sprain refers to pressure and swelling of the muscles, tendons and nerve roots in the lower spinal area which resulted form the origional fall.
Anything which damages or irritates the nerve will cause pain and discomfort in this region.
It is therefore very compatible with your history of a recent fall that you would have problems in this area.
Treatment obviously depends on the underlying cause.
