Head & Neck Pain with lose of Reflexes

I have suffered from lower right head pain for over 2 years. It's not a sharp pain but more of a dull annoying pain that seems to get worse when laying down at night. Even the lighest/softest pillow hurt. It feels like my head is made of concrete all the time. Sometimes i get shotting pain from the right back of my head by the ear and up to the forehead.
I have also had 3 spells in the past 5 years where i completely lose my vision for 5-20 minutes. I always get sick during this blackout but then when it's over i am fine and just have lower head pain for the remaining of the day. Not sure if this is connected.
My doctor told me it was migraines for 2 years and has now sent me to see a Physcial Therapy after I told her i can not handle it anymore. The PT thinks it may be nerve damage and requested an MRI. However, my doctor does not think this is the case and will not schedule a test.
Most recently i had a reflex test and they noticed that the left side is hypertensive but the right side barely moves at all.
I am not in pain all the time but i am tired of being light headed and uncomfortable all the time. The head pain makes me not sleep well and i am always tired and sore. I have changed my cubical at work, tried new pillow, etc and it just seems to keep getting worse.
Any suggestions?
