: Whenever I stand up, I get very dizzy and almost feel like blacking out for a couple of seconds.  I have tinnitus in both ears and I am lightheaded constantly. The back of my skull is also pulsating, to the point where my visual field is shifting left and right.  My MRI and blood work are normal. My ENG showed weakness in my left ear, and I have high-frequency hearing loss in both ears.  My ENT said he didn't think it was an inner ear problem (meniere's, benign positional vertigo).  Do you have any suggestions.  Thank you!

Thanks for the question. Your doctors have looked at some of the common causes
of your problem but there are other considerations. When patients become lightheaded
and feel like passing out when moving from a lying or sitting position to an upright
or standing position we tend to refer to them as have orthostatic intolerance. Blood normally
move to the legs when we get up and reflex mechanisms act to pump it back to the heart. If any
of thoses reflexes are not working then you may become lightheaded. Besides looking at the ears
one needs to evaluate your blood pressure during these events. Studying this can get very
complicated but we do have a syncope clinic at CCF run by Dr. Fouad in cardiology. Another
possibility is that the basilar artery (big artery that supplies blood to the back of the brain)
is narrowed and you are getting poor circulation. If you would like a referral to CCF to further investigate
these possiblities you can call the neurology department at 216 444 5559.
