Hyperactive Refluxes with Demylination

I recently had an mri for Brain C spine T spine, I have multilevel DDD especially C4-5 with disk protrusion effacing the thecal sac to a minor extent. No trauma or accidents occurred. I had experienced very strong cigarette smoke for weeks, nausea, ringing in my ears & cluster headaces, my mri showed gray areas of trauma, demylination, I am fatigued, all the time, headaches almost everyday back & neck pain. Recently had a QME evaluator tell he he was concerned about my  hyperactive reflexes with clonus in the lower extremities, and in the ankles he was amazed at them when they shook before relaxing. (but of course this is not work related per dr) Everything I read is terrible. My ankles are getting worse, hard to walk on in the morning.
What specialist can I see that will test my symptoms and give me an accuarate diagnosis. i am in my late 40's and want to get a handle on this before it gets worse.
