Is it TOA?

I have had pain and numbness in my hands for ten years. the pain goes all the way from my upper back through my shoulders, elbows, wrists and hands. The numbness is only in my hands and after a nerve conduction study one of my hands had a 40% loss and the other had a 60% loss. The results prompted a diagnosis of carpal tunnel syndrome and surgery was suggested. I declined surgery because both my radial and ulnar nerve are affected. My concern is still the pain shooting up to my elbows and shoulders with a constant upper back ache. I have lost most of the feeling in my hands and my grip is difficult to maintain. It was suggested that i may have thoracic outlet syndrome. Is this a possibility? Is there any hope for my hand function?
