Knee pain

I have had sharp pain in my knee for years.  The pain used to only come on a couple times a year and I told my doctor, but he wasn't concerned about it.  Now within the last couple of weeks (maybe a month now) the pain has gotten a lot worse in my left knee.  The pain is right behind my knee cap.  It is an intense sharp pain that comes on sometimes every other minute.  I can be walking, running, lying down, sitting down and it will still hurt.  The pain lasts for a day or two and then I don't feel anything for another couple of days.  Then it starts all over again.  I can't think of a time that I injured it.  I haven't been doing any strenuous work.  I was training for a marathon about three months ago, but I didn't have any problems at that time.  Does anyone have any idea what could be happening??
