Migraines with severe pressure behind ears

I am a 25 yr old female that has been diagnosed with migraines.  I have been on ultracet, bextra, tramadol, and durabac.  This pain used to be on the right front side of my head and now it seems that when I feel any kind of emotion, the pressure starts to throb behind my ears.  I have had several MRIs and Cat scans as well as blood work.  This pain has been here this time for over two weeks and nobody can seem to give me any ideas why this is happening.  I get dizzy, nauseated (sp?) and have had problems sleeping.  I feel like my last resort is to go to the ER, but it seems that they can't do anything for it either.  I am overweight and have had a hysterectomy (uterus only) and my gall bladder removed.  Any help would be greatly appreciated.  Thank you for your time.

Thank you for the reply Dr.  I live in Dallas Texas and have looked for specific clinics such as the one you mentioned and haven't had any luck.  I have decided to go to the ER today and see if anything can be done to help the headache.  I have been doing research on similiar symptoms and found that alot of the symptoms that I have been having sound like PTC pseudotumor celebri.  I understand that I am not a Dr. and cannot (and would not want to) make diagnosis but I will be asking my Dr. to perform a LP and see if there is any connection.  I am overwieight and realize that this may be affecting the headaches..Any thoughts?

Unfortunately I cannot give you a diagnosis for your headaches without seeing you
there may be several circumstances complicating your problem though
one is termed 'central sensitization' where you are hypersensitive to stimuli and this in turn can make headaches worse. The mechanism how this occurs is still being worked out, but several medications have been somewhat successful in reducing the effect, for inastance Gabapentin, tricyclic antidepressants

the other problem may be medication overuse - even several pills a week, and especially mutiple medications can worsen chronic headaches in the long run, this can also contribute to central sensitization. In particular opiaites like ultracet and tramadol can cause this. Coming off these medications and sticking to one or two effective medications under the supervison of a headache neurologist is reccomended. Anti-nausea medications like Metoclopramide can be very effective if nausea is a component in the acute setting according to a recetly published article.

For your current headache, which seems to be a prolonged severe headache, it may need to be 'broken' by some IV medications. Many headache neurologists have infusion suites so you don't have to go to the ER for this (such as at the Cleveland Clinic Headche Center)
