Mild cortical atrophy on ct scan-I just turned 29 years old

I'm not sure where to start with this. But I'm posting my question the best I can, while I'm still sort of with it.
Yesterday, I was at the er for horrible headaches that's been constant for a week, and horrible neck pain. I went to my local urgent care, and the doc told my fiance' (I wasn't with it at all, so he told me she talked to him. My memory of yesterday and the past week is spotty at best.) She told him due to the headache, neck stiffness and lack of cognition I needed to go to the er.
They did some ct scans, of my neck and head and compared them to one's I had I'm November because I see the nuerologist regularly for tremors along with muscle spasms and cognitive issues, but in november nothing was reported on my ct scan other than cervical kyphosis. Last night it was on the report that I have a partial fusion of the c5 and c6, and the head ct showed minimal cortical atrophy. I just turned 29 in april. Is this something I should be concerned about or bring up to my nueorlogist? It all started about 2 years ago. My eye site started declining rapidly, they put me on glasses it helped for a little bit but by 4 months I needed a different prescription. It's gotten worse, I developed horrible headaches, the tremors wich have gotten worse,it started in my right hand going into my arm and leg. Almost got into an accident because of the spasms in my right leg, and when it locks that time it locked on the break, and it was icy out. Anyhow, around that time I noticed my cognition declining, my memory has gotten much worse. And I'm not with it most of the time anymore. I can't recall things like I used to. Could this all be related to what the ct scan said?
