Multi-infarct Dementia (MID)

I have been diagnosed with MID caused by multiple thrombi emboli from an
an aortic valve prosthesis (Bjork Shiley Spherical Disk). The prosthesis
has been in for 21 years and I have been on anti-coagulation therapy
(coumadin)since then. I have recently been placed on anti-platelit medication
as well. Even with good prothombin control I get TIAs. This was the second
AVR; the first being a homograph which was implanted 7 years prior to the
prosthesis.I am told that surgery is not a viable option.
Symptomatically I have degradation of recent memory and have bi-lateral
tremors (appear to be intention tremors). Mysoline appears to reduce the
severity of the tremors. There is no familial relationship that I know of.
My question is: Is it possible that these so called intention tremors
have an etiology or are due to the multi-infarcts? Any comments would be
appreciated.
