NEED ADVICE IN GERMANY - AMERICAN SOLDIER IN CRITICAL CARE UNIT

All, I am Russell Noble, I'm a licensed Nurse - Paramedic in Missouri, formerly of the Jewish Medical Center / Barnes Medical Center in St. Louis. (CEN/RN EMT-DM)

I'm at the hotel Admira in Weiden Germany 961 / 48090 room 205.

An American soldier stationed here who is a close friend and fiance of a co-worker was undergoing laproscopic surgery for a gall bladder removal in Weiden. The closure of the blood supply to the gall bladder failed (reason unsure) and the patient began to bleed internally.

Bleeding was not noticed in time and the patient went into bradycardia and their blood pressure dropped to below 90/p.

They arrested and were transfered to another hospital in Weiden proper with a decent CCU. The patient spent over a week on respiratory support. Exact medications are unknown but included large doses of opiates.

The patient had a lowered blood pressure with reduced sa02 levels for an undetermined / unavailable period of time. Initial Cat Scans showed no physical adnormalities. The patient experienced twitches semilar to DT's or other withdrawl (withdrawal) symptoms when opiates were reduced.

Patient has been extubated and is breating on his own on a nasal canula at 6lpm, with an NRB 1 hour a day at 15 LPM.

Patient has signifigant third spacing in all extremities. Patient has acceptable but high systolic pressure. Pulse is ranging 70 to 90.

Patient is not communicating verbally now, however am looking for advise on good stimulation for patient (music / etc) and other advise. Have communication issue with German Medical Staff.

Talking to your friend, prompting him to talk, and spending time with him is the best therapy. A speech therapist evalustion and treatment may also be helpful. I hope things improve, just keep working with him. Good luck.
