Headaches/migraines - pain behind left eye in particular/base of skull pain...
Severe neck pain

Extremity spasms, cramping, and loss of sensation legs
Lower extremity - wasting
Facial numbness - right sided prn
Extremity - partial paralysis arm/leg, comes and goes
TN symptoms - in remission for 2 years on Neurontin

Ankles - 6 sprains this year with falling
Fatigue
I usually have what I call relapses seems to be 3-4 times a year, which will start with fatigue, disabling lifestyle, headaches with neck pain, increased dizziness, blurred vision, pain in my entire body (bones)like I have the flu etc. Episodes last usually about 6 weeks or so.
This is a pattern which I have documented now for the past 3 1/2 years and felt I should further reiterate this with a second question. My PMD told me this will never go away.
Should I take all my personal diary information to the Mellen Center along with all of my records including of course my scans, reports etc.? I just don't want to give an impression that I am crazy and a hypochondriac or something in showing my diary especially.
