: I have been experiencing dizzy spells for the past 2 months such that at certain times (not always), when I turn my head a certain way I get dizzy.  There are some days where this sensation is very noticeable and others that I feel almost fine.  I've also noticed over the past few weeks that my right eye will start twitching for a few minutes and then subside only to start a few minutes later.  I have been on medication (Meclizine, Claritin D) to alleviate what my internist thought was an inner ear infection, but neither drug helped.  I thought these symptons could be related to stress as my job has been very busy over the past few months.   Before I see a neurologist, do you have any thoughts?  Most appreciated for any insight you might have.

Dear Scott,
It sounds like you are describing a condition known as Benign Positional
Vertigo (BPV).  This characterized by brief episodes of vertigo or
dizziness with changes in head position.  Patients have described the onset
when arising from lying flat and when reaching to grab something from
the top shelf in the kitchen.  It is by far the most common cause of vertigo.
BPV is usually due to debris, often calcuim deposits, in the semicircular
canal of the inner ear.  Different positions will cause a shift in the
calcium deposits which may irritate the vestibular nerve, resulting in brief
episodes of vertigo.  The condition is benign and most people improve
with head maneuvers that can be done in the office setting.
There are a few other conditions to consider in your situation, but BPV
is most likely.  You should visit a neurologist for a complete evaluation.
The Cleveland Clinic would be happy to assist should you so desire.  An
appointment can be made by calling 800-223-2273, ask for the neurology desk.
Good luck.
