This could be mastoiditis, cervical lymphadenopathy or parotitis.
Have you seen an ENT specialist or your doctor for his symptoms? Does he have any associated symptoms of pain, discharge, fever, etc?
'Mastoiditis is an infection of the mastoid process, the portion of the temporal bone of the skull that is behind the ear. It is usually caused by untreated acute otitis media (middle ear infection) .'

Is there any kind of discharge coming from the ear? Does he have any vertigo or nausea? The pain may be there inside the ear and could be caused due to wax in the ear or a bacterial or fungal infection. The most common cause is wax in the ear. The wax can be easily removed with some ear drops, or in extreme cases could be removed with a forceps by an ENT surgeon. Hence, please consult an ENT specialist to ascertain the correct cause and treatment.
