Pain when working

Hi i hope i placed this in the right spot, if not in sorry, so inm a floor installer, like carpet, tile, carpet tile, vct, you geet the idea. so im union and im almost 10 years into my 30 years to retire. i also am active, fish ans resrore old bmx bikes.well im a righty and a while back i got laid off fot the first time ever, then i crush the bone on my index finger next to my thumb, (my trigger finger) right before it meets my middle knuckle. they cut a piece of bone off the back of my hand and screwed it into the finger then conected my knucke together, well i still had insurance and it covered the surgey. therapy was almost done and my insurance is about to run out. therapy was almost  over so i didnt go to the las few apt's because its a hour and a half drive/parking/gas/co pat ect. sorry to ramble just want all the facts in here so people understand....

so i can bend my finger so it just touches my palm, its always uncomforable but im getting used to it for the most part as much as i can. they said it will always be swollen and never be the same, plus it will always hurt a little. im still not back to work. my union"s slow right now. well since ive been doing yardwork, paddling the boat, painting and putting stuff together for our new baby comming, swimming ect. after a while it hurts. i had a long day the other day doing all sorts off erans and stuff. it aches after a few hours of working. today doind chores to. im s ure my job will really be tough when i go back,hammering/lifting/istalling, loading trucks, ect

so what should i do?? what are my options? should i see my primary doctor and ask him? i heard about pain clynics, go back to the surgeon wich i dont really want to do. i always complained i had pain at different time.  now the pain now is about a hour into labor but will kinda hurt all night. when i go back i know it will be bad. im trying to change trades but its not easy, we all know that.

what should i do. please any ideas? thanks in  advance
also my keyboard buttons are sticky so sry my typing is extra bad lol
