Permanently dizzy and off balance

I've been permanently dizzy and off balance for two years (with some fatigue). Had 2 MRI's and an MRV. All the blood work. No diagnosis. I also have a strong pulse that I feel in my head and body. I have some sore spots on my scalp and my vision seems off (not crisp). I also seem to have brain fog, finding it hard to concentrate. Any suggestions would be appreciated.

I should also mention that I've seen an ENT guy and all was fine with ears.
