Phrenic Nerve Injury

Is it common that a phrenic nerve gets nicked, damaged, causing respiritory distress due to anestiologist giving a second block during shoulder surgery due to first block not working properly (patient still feeling pain)? Can it have lasting problems? Are there any tests that can be done to check for severity of damage?
