Question about my father...

I am beginning to sense that something is off with my father.

He is 64 years old now, and has always had a problem with alcohol. My family and I have attempted to get him help in the past, and he always ends up relapsing. We have always had issues communicating as well.

With that being said, I have noticed behavior changes and mental ticks that seem to be irregular. These include, but are not limited to:

He used to have a passion for cooking, but somewhere between the downward half of his 3rd marriage and now he has stopped cooking.

He used to be relatively obsessive with cleaning, but now it seems he does not mind things being a little dirty. I have not unpacked from school and he has not said anything, the stove is very greasy. These are two things he would never let pass as close as 2-3 years ago.

His memory has always been relatively poor. However, recently we have been having the same conversation 3+ times a day. He has asked me constantly what day I'm leaving for certain places, and then at times will rephrase the same question and ask again.

Another thing that seems to has me very concerned are his eating habits. Every time we go out to eat he finishes less than half of his meal and takes it home, and when we eat at home he offers to clean the place and "continues to eat" once I have left the table.

He used to rarely spend time in his room only to do work on his computer (which has increased lately), and when he was in his room the door never shut. Now I would say for the larger part of the day he is in his room with the door shut.

There is a large feeling inside of me saying that this is his alcoholism/alcohol abuse getting worse and there are no other neurological disorders either. However, I am worried that it could be any combination of alcoholism/alcohol abuse, depression and/or Alzheimer's.

I appreciate any words of advice or points in the right direction that I can be given.
