Hi CCF,  I recently found out my MRI Brain Scan has gotten alot worse. I had a Cine Study done for CSF Flow. My doctor told me that my CSF was 100 % occluded to my brain because of Arnold-Chiari Malformation type 1.  Can you explain if this would be considered Hydrocephalus?  They are scheduling me in about three weeks for brain surgery for craniectomy to relieve pressure on my brain stem.
Also would like if having ACM that a lumbar puncture would make it worse?  Should I avoid having one done?
Thank you for all your help!
Sincerely, Anneliese Reitz

Dear Anneliese:
Sorry to hear about your severe Chiari malformation.  It may or may not cause hydrocephalus.  Most likely it will because the restriction in flow decreases/restricts the flow of CSF therefore causing increased volume of fluid the venous system has to try and absorb.  However, it may not, it would depend on the ability of an individual patient to absorb the fluid.  My thinking would be yes it would cause hydrocephalus (increased intracranial pressure).  No, a lumbar puncture would not help relieve the pressure because the fluid in a LP would drain the CSF from the lower spinal cord and not where the pressure is located (in your head, since there is likely little communication between the lower spinal cord and your head).
I hope the surgery goes well.
Sincerely,
CCF Neuro[P] MD
