Is it common practice when removing a disc and using a "cage" to enter through the abdomen?  This surgery sounds scary to me.  What is the advantage of going through the abdomen?  Also, if the L4/5 and S1/L5 discs are removed and replaced with "cages" and bone graft from the hip for a fusion, will movement in the back be limited after the surgery has healed?  I have been told that I have a tear in the outer area of the disc (ligament?) that will never heal.  (I had a discogram/discograph last July with a positive reading.)  Is fusion the only thing that will take away my back pain?

A cage used for fusion in the lumbar spine may be placed from an anterior or posterior approach with equal efficacy.  Most surgeons have an approach that they prefer for various reasons.  
Fusions are being done for 'discogenic' pain, that is, pain elicited on discogram, with good success.  On the average, about 70% of patients will find relief of pain after a fusion in such cases.  It is important to know that a fusion is likely to help your back pain, but not guaranteed to do so.  You will lose a little bit of mobility after a two level fusion, but it won't keep you from doing most things.
Speak to your surgeon about these issues for a clear understanding of the risks and goals of a fusion.

I had a 2 level fusion @L4/5 and L5/S1 and I was told success rate was 85%....but seeing another doctor, I was told the rate was 60-65%...and I ended up needing a posterior fusion with rods and screws.
this is not to say that it would happen to you....but my source for the second opinion was a doctor @ Jefferson in Phila...who is internationally known...Dr Vaccaro, @ the rothman institute.
I still am having problems, too....3 surgeries in 5 months has irritated alot of nerves and noe the thought is I have RSD....
so just be informed....and trust your doctor...good luck
