Dear Nidal:
A myelogram is an invasive test whereby a spinal tap is done and  
dye is injected in the thecal sac (the sac the surrounds the spinal
cord). The dye is allowed to circulate around the spinal cord and then
X-ray images are taken via conventional Xray methods as well as a CAT scan.
This allows excellent visualization of the spinal cord, nerve roots and
surrounding structures (such as discs, scar tissue and normal or abnormal
bony structures). Most neurosurgeons feel that a myelogram combined
with a CAT scan of the spine is in many aspects superior to an MRI
especially in areas where MRI scans are unable to delineate the
anatomy as well.
The downside to a myelogram (as compared to an MRI) is that it is invasive:
That is you have to have a needle placed in your back for a spinal tap and
this is somewhat uncomfortable. Other risks (although very minimal in the
hands of an experienced radiologist) include possible injury to the spinal
nerve roots and allergic reaction to the dye. Overall the risk is however
very small and the images are very helpful.
