I was diagnosed several years ago with

Radial head injury that comes and goes which can be quit severe at times. Recently I started to feel a tingling in my right arm, almost like cold water going over it. I have seen an eye doctor who refered me to a neurologist. The neurologist did a spinal, MRI and EMG of my arm, all negative. He put me on verapamil for several weeks which brought no relief. Now he is suggesting I see a neuro-opthamologist? Is this the right next step? Could this be Migraines or sarcoidosis related. Any input would be valued. Thank You!

Sorry about your sarcoidosis. In the face of negative exams and labs, on the surface it does sound like migraine. One should see changes on the MRI and some postive lab results from the CSF to indicate that your sarcoid has spread to the brain. However, without a family history, a clear signs of other symptoms of migraine, it is not a firm diagnosis. What you describe is compatible with several migraine variants, one of which is occipital neuralgia. The treatment is indocin, and if it doesn't respond than it is likely not this entity. You could also have another type of variant of occipital migraine. Just to be sure, I would see the neuro-opthalmologist and rule out any problems from his/her standpoint. Without seeing the films, lab results and doing the physicial neurological exam myself, I really can't make a firm diagnosis. But, if the neuro-opthalmologist gives you a clean bill of health, then I would see if this isn't migraine variant.
Things that are episodic we usually think of epilepsy and migraine. What you describe is not epileptic in nature, so I wouldn't consider epilepsy at this point.
