I am not a doctor, but have had two back surgeries for symptoms similar to what you describe (minus the testicle thing.)  Go to see a neurosurgeon who has experience with back problems.  It sounds from the pain you describe like sciatic nerve involvement.  The nerve may be influencing the testicle thing, but I have no idea about that part.  I had a myleogram and CAT scan which helped to identify what was involved.  Good luck to you.
