Shoulder injury, what is it?

Hi, I hurt my shoulder about 2years ago while swinging a baseball bat. After I swang the bat (im a righty) my left arm went too far back with the bat, and it felt as if my shoulder was riped off. I feel the pain in the middle and on the top of the shoulder. I love to walk, but I can only do so much because my shoulder begins to hurt after about 30 mins of walking. While sleeping I cant lay on my left shoulder because it would start to hurt. What type of injury do you think this is?
