Small vessel disease on a 22yr old's MRI - cause for concern?

10 year ago - at 22 yrs old, I experienced 2 episodes that were diagnosed as migraine w/aura - numbness/tingling in left arm, tunnel vision, slurred speeach followed by a headache.  My birth control was dc'd.  I had one more episode when I was pregnant w/ my first child @25.  I had an MRI w/the first episode and was told it was normal in 1997.  I just rec'd a copy of the MRI (insurance purposes) and it doesn't look normal - "there are five approx. 3mm foci of increased T2 signal, predominatley in the subcortical white matter of the right frontal lobe. The largest lesion is about 2x5mm, which lies close to the right anterior forcepts of the corpus callosum.  No actual lesions are identified in teh brain stem, corpous callosum, or posterior fossa, and the brain parenchymal is otherwise unremarkable." Impression: "A small number of nonspecific punctate white matter signal abnormalities are demonstarted that are somewhat more in keeping with small vessel disease than other causes such as demyelination, given their predominant subcortical location."  Should I be concerned?  Is there need for a f/u MRI?  My BP is normal, a non-smoker, not overweight.  Am I at increased risk for dementia or stroke?

Migraine is a disorder of spasm/relaxation of small and larger vessels.  That's how the aura's, sumptoms and pain are caused.  They often are seen on MRI with exactly the lesions you describe.  Sounds to me like your MRI is consistent with having migraines, so I think you can relax.  Your neuro will explain further.
