Sudden sporadic headaches-related to spinal tap?


  In November of last year, I experienced a severe and lasting headache accompanied by neck stiffness, only relieved when I would lie down.  It felt and acted just like the week-long spinal headache I'd gotten after a spinal tap I had several years ago (for menningitus.)  My doctor diagnosed me as having a virus, as many other patients were coming in w/the same symptoms.  When my pain didn't lessen after five days (I was by then on Darvocet), he had me go in for a spinal tap.  The tap was negative and the neurologist felt that my pain was probably "neuromuscular" in nature.  He just told me to rest. I did as he said, and got no relief or lessening of symptoms.  I went to a chiropractor, who took x-rays and got all excited about how my neck was curved opposite of the way a normal neck curves.  He began giving me adjustments.  I was still experiencing enough pain, that for weeks, I couldn't sit or stand up for any length of time.  Over Christmas, it seemed to lessen in severity.  But then, just as I thought I was okay, I played a round of golf...and had such severe pain I had to stop and immediately lie down.  I got an MRI in mid-January which showed that I still had spinal fluid in my brain, from the tap.  But I was starting to feel better.  Now I've been getting sudden and severe stabs of pain, again on the top of my head, when I strain (go to the bathroom, cough, laugh) and when I bend down.  Could I possibly still have a small leak where my spinal tap was given?  I am getting worried and my doctor doesn't seem to have any answers (last time I saw neurology, they said they think I have migraines...because of my past headache history - but this is much different than anything I've experienced before).  Any light you can shed on this would be greatly appreciated!
  FYI - I am a healthy, active 31 yr. old female.  I do have allergies and asthma and am on several meds for those.

Thanks for the question and sorry about the delay in answering. Post spinal tap headaches have been known for many years and clinically the patient is okay lying down but has increased pain with standing. Some patients can develop a persistent leak and start to suffer what we call low pressure headaches. These are again mostly posture related. If the headaches are very bad a blood patch (your blood is injected into the spinal fluid to close off the leak) can be given. I have treated several patients with high dose oral caffiene for low pressure headaches with soem relief. Patients can develop other headache syndrome and it is possible that you have developed exertional headaches. It concerns me that you have increased pain with excertion and  this may represent increased cerebrospinal fliud pressure? You said when they did the MRI you still had fliud in your brain? I think you need to sit down with your neurologist again and discuss your concerns. Good luck
