TLE OR PSYCOGENIC SEIZURES-CAUSED BY A BRAIN TRAUMA?

  I am a 39 year old female who became overcome with a paint thinner type spray containing xylene/toluene.  I lost conscisnous and fell to the floor. Witness say my head "bounced off the floor twice.  I have had two EEGs' One that stated an unusual appearancewith a paroxysmal character to it,which is rather atypical etc.  My Neurologist believes that since my second EEG read normal that I have Pseudo Seizures or Psychogenic Episodes.  I have gone from 3 to 5 seizures a day to 1 to 3 a week with Dilantin.  My question is : if it is caused by an Emotional overlay, could the toxin or the blow to the head have caused it?  I didn't have this before it happend.  I know I have given only a sketchy summary, but it's a very complicated case.  I appreaciate any insight you may give me.

It is very hard to tell if your spells are seizures or pseudo seizures.
In general, the diagnosis of pseudoseizure is made after a typical spell is recorded with EEG at the same time without any abnormalities on the EEG. This is usually done in the hosptital where the patient is admitted and hooked continuously to EEG and video in order to record some seizures.
The response to Dilantin may be suggestive of real seizures, but it is hard to be sure based on the little information I have.
I am not sure what the first EEG abnormality is.
EEG can be normal even if the patient has epilepsy.
I would suggest the following:
1. Continue Dilantin.
2. Continue to take  all seizure precautions (consult with your local doctor concerning the driving restrictions in your state)
3. Get a second opinion with review of both EEGs. This may be arranged at the Cleveland Clinic Epilepsy Center by calling 216 445 5559 or 1800 CCF CARE
4. MRI of the brain may be needed.
