Tourette 19 year old with new symptoms

My boyfriend has been diagnosed with tourettes. Hes lived with it for about half his life, and generally has very good control over it. His tourette's is much more than just a tick, when he loses control its usually resembles a seizure. His whole body will spasm, and on several occasions he's dislocated joints. Hes tried medication and none have really had an affect on him, so now hes given up on seeking any help from doctors.

This brings me to our current situation. In the past month or so he has developed a new problem all together. It seems completely seperate from his tourettes but still involving the neuro system. It happens randomly and to any part of the body. Unlike his tourettes, its isolated to whatever area it starts in. It brings him an imense amount of pain. Usually resulting in him thrashing, crying, and screaming out. Theres not any physical sign of pain, unlike his tourettes with the spasms. If it weren't for his cry of agony, you couldn't tell he was in pain at all.

I can't physically touch wherever it is that hurts him, or else it causes him more pain. Genreally, these fits last for about 20 or more minutes, which when suddenly decrease in the amount of pain, and afterwards causing him to pass out.

He's refusing to see anyone, due to past failures with his tourettes, and its causing him to lose his will to keep living. I don't know if anyone can help, but any sort of information would be very helpful.
