Treatment options for Thecal Sac narrowing, Marrow Edema, and Annular bulges??

Hi, could you please give me any information as far as treatment options go for the following MRI reading?

1.  S1. Disc dessication (desiccation), annular bulge,
mild bilateral facet arthropathy. No significant stenosis as seen without contrast.

2. Schmorls node inferior endplate L1 and superior endplate S1 with mild marrow edema.

3. Annular bulge at all lumbar levels, with mild thecal sac narrowing at L1-L2,L2-L3,L3-L4, and L4-L5. mild thecal arthropathy at L4-L5 and L5-S1.


My boyfriend is in a lot of pain and is not really able to ambulate like he should.  He is 29 years old and is experienceing what I believe to be sciatica because of the pain shooting down the back of his leg along with some incontinence.  If you could give me best and worst case scenarios as far as treatment options go.  He is not able to get in to see a physician for another month because of insurance issues.
