Have you tried a Chiropractor. They are the only doctors qualified to do chiropractic adjustments and it sounds like one could really help you.
I truly understand what your going thru. I had a PA at work try manipulation on my lower back and neck area. After he did that I was put in the hospital. This has been over a year ago. I have been given every narcotic possible but I'm afraid of them. I take them and feel very weird, this freaks me and my family out. I only take a ton of tylenol now. I don't feel like a crazy person anymore. It sounds like we have the same symptoms. My muscles twitch all over. I have a pain in my lower back area that it feels like something is slipping out. And when it does I can't move. People and the Dr.'s think I'm crazy when I tell them this. I also, have a excruciating pain in my butt and it goes down my right leg and into my right foot. My calf feels like it is on fire. My right leg is numb and tingly all the time. The color of my leg is purplish and has white spots all over it as well as my arms. I tell the Dr.'s but they don't seem to care. I had a MRI of my neck and it showed a herniation and the Dr.'s state that it will go away. Well, its to the point now that I have migraine headaches all the time and is nauseated and very dizzy. My muscles ache really bad. The cold is really bad. I wear my winter coat and mittens in my house. My family is about to commit me. My Dr.'s are sending me to a Arthritis specialists. I don't know why. Well, I appreciate your ear. Sorry to hear about all your symptoms, the most concerning of which is the urinary problem. Cauda equina syndrome is not something we take lightly as it can leave some dramatic long-term problems with bowel and bladder continence. The other symptoms you report are also suggestive of a nerve root problem. An MRI is pretty sensitive in picking up disc abnormalities, but it's possible with all the hardware and previous surgery, it may be difficult to visualize the anatomy of your lumbosacral spine and the nerve roots exiting from the spinal cord. I recall from our previous communications that you were going to see a CCF neurosurgeon and possibly get a myelogram for better visualization. This sounds like a good start. I certainly would not dismiss these symptoms as they could potentially get worse. Neurontin is a good drug for pain relief but not a cure-all. Talk to your surgeon and let them know what's going on.
