UBO, same place, 5 months later

Good Morning. If you note my nickname, tiredofnoanswers, you may discern the obvious. Now, a small history concerning my neurological issues:
I am 43 y.o., female, smoker. I have a hx of Migraines, first diagnosed in 1989, after the birth of my 3 of 5 children (my 1st epidural block). I have a hx of FMS & CFS, probably dating back to the mid '90's, with the strongest "flair" occuring in 2005. I have had numerous MRI's & CT's of my brain since 1989, all showing no abnormalities. I have been dx with being Bi-Polar (1983) and Schizo-Affective Disorder, Bi-Polar Type (2005),Tremors (2002), RLS/PLM (2006), TMJ (2006), periodic Dysphagia (2007), a small right nasal cavity polyp (CT of face, 2007), Dyspnea (2001) associated to Moderate Aortic Regurg.(2001) & Aortic Arch Aneurysm (Stable) (2001), and Mild Cervical Scoliosis (possibly due to retrolisthesis C3-C4, possible due to Osteoporosis) (2007).
On an MRI ordered by my "pain doc" on 9/14/07 because of more Migraines than usual, everything was normal except "a small UBO in left frontal white matter". The same results were shown on an MRI of 02/19/08, with the UBO in the same spot with "no significant interval change". My cognitive abilities have been steadily diminishing over the last few years, particularly the last couple months, and I seem to keep blaming it on lack of REM sleep.
My current and most bothersome symptoms are:
A) In two seperate errand trips while driving my car, I was overwhelmed with a need to stop the car, ask my husband to drive so I could lie down in the backseat as I felt I could not "function", period.  These trips were 8 days apart, about a month ago.
B) On Valentine's Day evening while preparing food, I was overcome with the above feeling of overwhelming exhaustion AND I got double vision, nausea, and the feeling of passing out. I had to lay down, which I did after taking my bedtime meds. I was asleep (?) within 10 minutes and did not stir for 8 solid hours, which NEVER happens, mostly due to pain.
The abovementioned ( A & B ) have never come on so quickly.  They were not associated with a migraine or anything else I have symptoms of.
C) I have been dropping things, every day, several times a day.
D) It has taken me about 1/2 hour to try to type this. I have to search for words, re-read to remember what I was trying to convey. My lack of cognitive abilities is worsening, it seems, every day.
(Right now, I can't concentrate enough to finish my current symptoms!)

I am (was) very intelligent. I know what many of the aforementioned dx.'s can be ruled out as a cause for UBO's. My question is, what can be ruled IN? I know my doc is playing the "watch & wait" concerning MS, but should I be worried?
