Dear Marline:
All hospitals, and specially neurologists, very frequently deal with undiagnosed problems. The better these neurologists are, the fewer of these problems remain undiagnosed! Unfortunately, neurology is a speciality replete with very rare/new/difficult-to-diagnose diseases. If the disease persists or worsens, diagnosis may get easier with time. There is no harm in seeking an Nth opinion if symptoms persist.
Your brother should go to a large academic neurology department with nationally renowned experts in every subspeciality of neurology to maximize his chance of getting diagnosed.
