What is grade 1 retrolithesis/severe right neural formanial narrowing

I am have had an Anterior Cervical Fusion of C4-C7 in 2001, over the years, have dealt with the chronic pain, until recently, no meds, messages, heat or cold tx helped. Went and had xray found out 1 of the screws (bottom rt.) of the 8 attached to the titanium plate is not secured in, and visibly seen on an Xray that is is coming out. I had an MRI done, and received report that I have Grade 1 retrolithesis and posterior osteophyte/disk complex, and mild canal stenosis and severe/right/mild left neural foraminal narrowing is identified!  I am an RN but cannot make sense of this.  I have not spoken with the Neurosurgeon yet, but at this time, I am not responding to any kinds of tx's I have been given, or medications!
