Whats next, Tried all else for back pain

I had a fall a little over a year ago and since then been having problems with back,hip and legs. I have been seen many times by many doctors and had a few xrays, ct scans and mri's done and this last one said the following:

DX:: LOW BACK PAIN W/ L LEG PARASTHESIA

MRI LUMBOSACRAL SPINE

PROCEDURE: The examination was performed with sagittal T1 and T2 sequences and an axial T1 sequence from L3 through S1.

COMPARISON: Examination of 4/37/07.

FINDINGS: The alignment of the lumbar spine is normal. The vertebral bodies are normal in height and signal intensity. The conus medullaris is normal in contour and signal intensity ending at approximately the L1/2 disk level.

The L1/2, L2/3 and L3/4 disks are normal in height, contour and signal intensity. No disk bulge or protrusions are seen at these levels. There is no central stenosis or foraminal stenosis. The facets are normal.

L4/5: The disk is desiccated but unchanged from 4/27/07. There is a mild symmetric bulge of the disk. Mild to moderate facet hypertrophy is present bilaterally. There is no central stenosis. There does appear to be mild bilateral foraminal narrowing.

L5/S1: Minimal disk bulge is present. The disk is normal in height and signal intensity. No central stenosis is seen. There is mild facet hypertrophy bilaterally. There does appear to be right foraminal narrowing due to a combination of slightly asymmetric bulge of the disk and some facet spurring. This has not changed significantly from 4/27/07.

IMPRESSION:
1. Degenerative disk with desiccation of the L4/5 disk. Mild to moderate bilateral facet hypertrophy is present. There is mild bilateral foraminal narrowing.
2. There does appear to be right foraminal narrowing at L5/S1 due to a combination of slightly asymmetric bulging disk and mild facet spurring.


And Doctor set me up to see a Neurosuergen for this. I also have done several different times physical therapy and this did not help also did ultrasound heat therapy, electronic stimulation of the muscles, traction and masage therapy and I still have the pain, only now I can't get comfortable for very long if at all. If I stand for more than 15 minutes or lift anything more than 10 pounds I hurt and even been on muscle relaxers, pain pills, celebrex and more and nothing helps, extreme cold or heat makes it hurt more. I have been off work for several months to deal with this and am getting frustrated cause I want to work. I work in a Printing firm in the bindry department and we have to lift up to 50- 80 lbs at times but usually less than 50 most of the time. I was wondering if there is any way to fix this problem so I can go back to work soon and to minimize the pain. And why do I need to see a Nuerosurgeon for this. I was Told my case was a special one so they had to try to get it done as soon as they could but I still had to wait a month cause of the need for the specialist by others too. This may also be hereditary for my parents have both had surgery for this too but they waited longer to deal with it. Also on dads side several other family members also dealing with back problems and two are in wheel chairs at age 50. Will this mean I have to change my career options I am hoping not I only had this job 2 years and love it and live for it, and it is great paying job with benefits too.
