I have poor balance, slured speech, congnitive skills are poor, muscel weakness,
what could it be

There can be several reasons of her slurred speech, lack of balance and muscle weakness symptoms although without being able to examine her I can not offer you the specific advice on diagnosis and treatment that she needs, but I would try to provide you some relevant information about her health concern.

Most likely your symptoms could be due any demyelinating or neurological disorder, injury to a nerve, pressure on spinal/peripheral nerves (neuropathy), vitamin deficiency that need to be evaluated thoroughly with the help of a neurologist. Neurological symptoms may include slurred speech, memory loss, vision problems, sleep disturbances, dementia, sensory loss in the limbs, dizziness, difficulty in walking /swallowing, loss of muscle tone, loss of fine motor coordination, and sometimes seizures.
