carotid artery dissection and tingling in hand

I have a dissection of the right internal carotid artery - about 6 weeks out.  I was just starting to get out and walk (the only exercise I can do now) and drive a bit, when a week ago I developed new symptoms - tightness in neck, a "lump" in my throat sensation (as if a huge pill was stuck in it), pressure in chest (esophagus area?), increased dizziness, faintness when walking, and pressure when swallowing.  Two days ago, I developed pain and tingling in right hand and some transient sharp pains in arms and chest.  About the time the former symptoms developed, my doctor had taken me off coumadin and switched to aspirin, and I had just finished a series of MRAs of the thoracic aorta etc. (which were relatively normal).  I have since switched back to coumadin in the hopes that the new symptoms would go away but they haven't.  My artery was near occlusion and perhaps the switch caused it to close off completely and thats why I'm experiencing new symptoms?  Any thoughts and do these new symptoms sound serious?
