cervical fusion

On January 12, 2011, I had cervical fusion on C3, C4, C5; cage, metal plates, screws and bone graft were implanted.  Now, 3 weeks after surgery my surgeon tells me that a bone has collapsed and screws are pulled down or sideways.  Immediately after surgery, the surgeon reported to my husband that everything looked fine, bones looked good.  What could have caused this?  I followed doctor's orders, didn't engage in any strenuous activity.  I did, however, drive and run a few short errands on the 10th day post-op.  After that, everything changed.  Muscle spasms, numbness in hand returned.  
I've had recent bone density scans and was pronounced fine.  I am not aware of any particular "event" that indicated this bone collapsing.  What could have possibly happened?
