   I was in car accident 1 1/2 years ago. C5-6 herniation and T11-12
  Herniation> Just had an evaluation and the ortho said I have 35%
  weakneess in R hand and radiculopathy R leg. How do you evaluate
  whether it is time for surgery. Already did all conservative modalities,
  except epidural injections. Do I get done now or get further studies,
  or really look at surgery. When do you have to get surgery. I really
  need to know botttom line, I am a nurse and know all the facts.

Thanks for your question.  One of the criteria used to determine the necessity
of surgical intervention is the presence/absence of focal motor symptoms.
You have already described a focal weakness in your R hand (although I am
quite curious in how your physician was able to give you such a precise
estimate of your weakness).  Another test that can both help in determining
the exact location and extent of the nerve lesion is an electromyogram (EMG).
You should have this test done by a neurologist who specializes in neuro-
muscular problem (preferably in a major academic/clinical institution).  The
peripheral nerves and muscles of the affected limbs will display very specific
alterations in the EMG which should help localize your lesion.  Please
discuss these evaluations with your physician.
