charcot foot

charcot foot

A 60 year old insulin dependent diabetic on dialysis for the past 4 yrs fell on the way to treatment and
developed pain swelling and erythema of the right foot.
Over the past 6 months the pt has developed Charcot type changes on radiograph despite immobilization.
The foot is swollen, deformed and she is unable to be bear weight.
The Charcot Joint destruction of the bones is ongoing and the local MD says there is no treatment other than immobiliztion.
Would there be any role for bisphosphonates or Prolia?
Is there any surgical treatment or ultrasound or bone stimulator that might help.
Do you recommend a neurologist see the patient or should she continue under the care of an orthopedist
Is there a specialist in the NYC area you recommend for Charcot foot.
I am worried that amputation may eventually be necessary in this patient.
Thanks for your input.
