chronic headaches/migraines

  One month ago, my 17 year-old daughter passed out, hit her head on concrete and suffered a severe concussion.  She had seen a neurologist previously for chronic headache and was diagnosed after an MRI with tension headaches and migraines.  She has had an episode of heart palpitations with what she described as heart compressions.  She normally runs a low blood pressure.  She has seen a cardiologist and he did an EKG (normal) and recommended a tilt test next.  She is plagued with continuous headaches daily, many of which are severe.  She has had 2 MRIs, EEGs and EKGs all of which were normal, for which I am thankful.  She has had Midrin, Imitrex and Nortriptyline prescribed, all of which can have side effects which can possibly be heart-damaging and none of which have been effective on the headaches.  Some days (especially mornings), it is impossible for her to function.  Any advice that you could give would be appreciated. Would you recommend a headache clinic at this point or staying with a neurologist?

I would reccomend that you try to have the best of both worlds, you should find a
neurologist who specializes in headache management and have all aspects of her
condition addressed.
Complex headache cases like this are best managed within the context of a
specialized headache clinic where the full range of options is available
and the physicians have experience in using the all of the drugs available
to treat chronic headache.
There are a number of other drug options available including drugs like
Depakote and Neurontin which were origionally produced to treat epilepsy
but are now found to have a role in the treatment of headache.
