You might have some kind of neuropathy going on. I have RLS and I also have cold areas on my lower legs and feet. When the doc puts a cold instrument on my faceFace pain, I feel it real well, but anywhere below the knee and on my feet, I barely feel it. My feet and lower legs get cold a lot, too. My neurologist, who treats me for RLS and my injured back, recently added neuropathy to my list of health issues. Now, also your symptoms could have something to do with diabetes, that can lessen sensations to the lower extremities, too. Ask your family doc to do an X-ray of your back, test you for diabetes, and ask him about neuropathy. He'll know what to do then.
