continuious headache

Dear Cleveland Clinic Neurology,

I'm a 30 yr old male with a 6 year history of symptoms similar to new daily persistant headche, however I have a 3.5 cm x 2.5 cm bone tumor as seen here in my MRI posted online, please look:

My symptoms are constant 24/7 pressure behind my forehead and in the bridge of my nose, with a constantly blocked feeling to my nose.    Also, a stiff feeling in my forehead when I try to raise my eyebrows.    Often, when the pressure is the worst,  I get severe vertigo when laying down only and even more pressure in my nose.

A doctor has proposed a supra-orbital nerve block to test what will happen,  as the pressure headache has been very resistant to drugs of all types.

Do you think the tumor is causing my symptoms and would a nerve block help with dull pressure in the head?

Our role on this website does not include reviewing MRI images. Without the ability to examine you and obtain a history, I can not tell you what the cause of your headaches are however I will try to provide you with some information.

Pain due to supraorbital neuralgia (irritation of the supraorbital nerve) is usually associated with burning scalp pain on the side of the irritated nerve. It does not typically cause bilateral forehead pain associated with stuffy nose. However I can again not comment on whether or not supraorbital neuralgia is a cause to your particular symptoms.

As you know, daily headaches of 6 years duration could have multiple causes: they could be primary or secondary to another disorder. One of the most common causes of daily headache of such a long duration is often due to medication overuse, or what is called rebound headache: the medications that were initially taken for a headache then start to cause the headache. This is particularly true when excessive tylenol, ibuprofen/other NSAIDs, or narcotics are taken daily. Other potential causes include pseudotumor cerebri (which usually occurs in obese women), intracranial hypotension (a low CSF pressure, which unlike in you causes more pain on sitting or standing compared to lying down), chronic tension headaches, and others.

Given your long history of headaches, you may benefit from evaluation by a headache specialist in a specialized headache center if you have not already done this.
