cortical atrophy

My Father is 74 yrs old, has diabetic ,(he was a smoker up to 60 yrs old)
because of small wound ,( january 20th ) remove 2 toes in one of his leg,  with antibiotics heeling slowly,
last Saturday doctor put LUPINEM 500mg injection to him,(fist time they put this)
After 2 hrs he got fits, nurse checked his sugar level, very low , and they gave some sugar.
Night again they put this injection ,after 2 hrs again fits came ,then he is admitted in ICU
DOCTOR took CT SCAN (head),
This is from report :

1.age related cortical atrophy
2.Atheroselerotic, Demyelination in Bilateral cerebral white matter & right basal ganglia
3.Lacunar infarct in both basal ganglia

Leg report:

Fluid collection suggestive of abscess in deep planta aspect of the foot.

2 days his behaviour is little different, lack of memory loss(now ok) ,
Now he is shifted from ICU to room , last night  they again put this injection (LUPINEM 500mg) he feels tired, then check sugar level ,its very low.then put glucose,night he sleep well.

what the report says? after put this lupinem 500mg ,why sugar level reducing,? what will happen if the fluid collection is in foot? we are worried about our father.

Dear Doctor,
I need your help to clear my doubts,

Now my father is discharged from hospital, good improvement of wound heeling
But now the problem is, after he got fits(due to antibiotic Lupinem -500mg,glucose level reduced) lot of changes in his behaviors  ,some times he speaks normally, some times he never speak, that time doctor said that this is due to fits,
Its called delirium , after some days he will be normal ,like that he said. Then I read  so many delirium articles from net ,they said if the cause is an infection, antibiotics may solve the problem within a few days.

But now almost one month , he always sleeping, not eating food properly(he never open his mouth) , if we ask anything some times he is speaking, (now he is too week ,he can’t get up from bed without others help.)

I understand, for his age we can not expect more, but before this operation he walk with stick very slowly(only house), but he speaks well, and always fight with my mother, but now totally changed that’s why we are worried,

Before delirium (ie after removed toes) he was normal, is it curable doctor? What other things we have to do for him to make his normal condition?

Please doctor this so big to read ,but I need your answer

Hi,

The doctor is probably suggesting this because of your father’s age and also because he is diabetic. Diabetes delays wound healing.
The other option is local radical debridement and post debridement management. Leg pain could be because of pus and also there are chances of involvement of other toes as well. I would suggest you to consult an infectious disease specialist along with a vascular surgeon and an orthopedic surgeon as it will require the expertise all three in order to manage this abscess.
Crocin is ok to take as long as it does not exceed the total recommended daily dose which is about 8 tablets in a day.

one more question

you said They are treated with surgical incision and drainage.our doctor also plan to do this,but what they are telling suppose after surgery if the wound not  heeling,again it is the big problem,so we dont do anything now, if he has severe problem then we will do like that they are telling.

past 3 days my father has leg pain(toes wound heeling) per day he is taking 2 crocin tablet,
my question is
1. leg pain is because of this pus  or removed toes?
2. if we are not doing "surgical incision and drainage" this pus formation affect other toes?
3. taking more crocin no problem?

Hi,

Thanks for the information.
Lupinem contains Imipenem and Cilastatin. Imipenem can cause seizures so probably this is the cause of fits that your father has been getting.
It is important that you discuss this with his treating doctor because if he is getting fits every time he takes the infusion he may need to be put on alternate medication. Hopefully since he is taking eptoin the seizures will not recur.

Dear Doctor,
The medicine name is Lupinem -500mg(1 bottle),from that they take 10ml Lupinem add with 100ml saline water, they put this injection morning and night.
From yesterday they put this injection morning once, in addition they give Eptoin-100mg tablet 1 number in morning and 2 tablets in night time.
Yesterday night he is not sleeping because of back pain, some times he is telling leg pain.
Please give us some more explanation (advice), thank you

Can you please spell the name of the medication again that he is currently taking?
Plantar abscess or foot abscess are seen in diabetic patients on the sole of the foot. Most of these abscesses occur due to infections at the sole of the foot. They are treated with surgical incision and drainage and the method used for incision is known as hilton’s method.
