diffused axonal injury

Hello Doctor..this is regarding my relative who has suffered a TBI 6 months back diagnosed as diffused axonal injury.Clinically he is showing very slow cognitive and motor progress.recently he underwent burr hole surgery for subdural haematoma.what are the chances of recovery after this surgery?

Hi there.  Diffuse axonal injury means injury not limited to a focal area but over a more widespread area.  This is one of the most common and devastating brain injuries. Severe diffuse axonal injury is one of the leading causes of death in traumatic brain injury sufferers. Apart from immediate measures to reduce brain swelling which can cause additional damage, course of steroids, to reduce inflammation. If he has sustained mild or moderate diffuse axonal injury, the rehabilitation phase will follow once he is stabilized and awake. Multidisciplinary approach of therapy including doctors,’ nurses, physical and occupational therapists, adaptive equipment training and counseling would be involved if he is conscious. Recovery is possible in mild to moderate forms of diffuse axonal injury whereas most of the patients with severe injury will remain unconscious. The burr hole was done to control the pressure symptoms from increasing  subdural hematoma.  You need to discuss your relative’s condition and prognosis with the treating intensivist and neurosurgeon. Thanks sir..he has suffered a severe DAI.but now he is conscious and following light commands.His right side of body is not working at all in spite of regular physiotherapy and cognitively there is very less response.can you please suggest some good rehabilitation set up in delhi or elsewhere in india for such patients?is it possible to take him abroad if we can afford that?usa hospitals mostly require health insurance,is there any method by which we can work that out?
